The highlights of this week's edition are randomised controlled trials on extracorporeal CPR for refractory out-of-hospital cardiac arrest & higher protein dosing in critically ill patients with high nutritional risk; systematic reviews and meta analyses on dexmedetomidine for the prevention of delirium & extracorporeal cardiopulmonary resuscitation; and observational studies on a wearable cardiac ultrasound imager & diastolic blood pressure thresholds during pediatric cardiopulmonary resuscitation. There are also guidelines on pediatric Acute Respiratory Distress Syndrome & surgical management and mechanical circulatory support in high-risk pulmonary embolisms; narrative reviews on liberation from mechanical ventilation in the cardiac ICU & biventricular assist devices and the total artificial heart; editorials on liberation from continuous kidney replacement therapy & advancing precision-based antimicrobial dosing; and commentaries on whether nephrologists or intensivists should manage kidney replacement therapy in the ICU & conditional power: how likely is trial success?

We've been quietly working on the CCR23 programme over the past several months and hope to start announcing the provisional trials and faculty this week. As usual, we'll host major trial results, review some of the best trials of the previous 12 months and have expert panel discussions on various aspects of clinical trials.
